Responsibilities:
The Health Records Management Specialist II will handle external and internal calls and requests regarding release of Protected Health Information.
Provide customer service via e-mail, fax, mail, telephone to satisfy requests or complaints.
The Health Records Management Specialist II will complete and submit patient forms including disability, educational/LearnWell, FMLA, and leave of absence in coordination with other Caron departments in accordance with established guidelines.
Process legal matters regarding PHI including power of attorney, subpoena, court order, and Marchman Act documents received as well as requests from probation/parole departments, state and federal agencies, judges, and attorneys.
The Health Records Management Specialist II will handle requests for PHI from health insurance companies in accordance with applicable state and federal regulations.
Complete requests and Discharge Summaries for the Healthcare Professionals Program; work collaboratively with the Health Care Professionals Coordinator and other unit staff.
The Health Records Management Specialist II will enter Letters of Medical Necessity in electronic records and provide to patient for insurance appeal purposes.
Print partial and complete electronic records for external use.
Record Protected Health Information sent outside of the organization in the Health Records Management Contact Log and/or HIPAA Disclosures Log.
The Health Records Management Specialist II will assist with annual and as needed update of Health Records Management procedure manual through review and revision.
Training of new staff within the department in policies and procedures.
Assist Caron staff in the handling of functions related to Health Records Management such as contract entry, completion of Consents to Release, etc.
The Health Records Management Specialist II will manage Health Records Management roles for all units and locations served by the Health Records Management Department.
In addition, the following Health Records Management Specialist I roles may be required for coverage or as part of daily role for all Caron locations:
Transcribe dictation through InScribe (including, but not limited to, History & Physical Examinations, Progress Notes, Psychiatric Evaluations, Psychological Evaluations, and Letters) in accordance with established guidelines.
The Health Records Management Specialist II will enter information into electronic records including, but not limited to, transcribed documentation, DSM5 Assessments, Treatment Plans, Personal Safety Plans.
Prepare unit reports of unclosed records and notify staff of needed documentation/signatures.
Completion/closing of electronic records in accordance with established guidelines.
Ensure that all documents, notes and records are completed according to standards.
Scan documents into electronic records.
The Health Records Management Specialist II will proofread and mail Discharge Summaries to referrals, physicians, aftercare providers, etc.
within established timeframes.
Research returned mail and update in database.

EDUCATION/EXPERIENCE
Qualifications:
High School education or equivalency required.
2-3 years of health records experience is required.
Extensive customer service experience, specifically with phone work required.
Previous experience with leave of absence or medical forms preferred.
If in recovery, 2-years of continuous sobriety is preferred.

Understand state (PA, GA and FL) and federal privacy and confidentiality regulations as well as CARF standards.
Must be able to demonstrate the ability to transcribe 50 words per minute.
Must have strong spelling, grammatical and proofreading skills.
Must be able to demonstrate an understanding of basic computer skills.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
